Solved filling out the domain form

I'm trying to get my own domain through forumotion and I'm having a little trouble filling this form out... the red circled areas are the parts I'm not to sure what they mean.

What does "name" "first name" and "address" mean, do they want my name for the domain name or the forums name? same with the addy, is it the URL or my actual addy?

That would be your First Name (Real Name) Address where you live and Name of your Web Site
