New user registration- spam

Can any one tell me how can I set "Textual Confirmation" on new user registration.Textual Confirmation asks newly registering user a question. If the answer is wrong, TC rejects the registration.I need that all protects because spam.Pls help.

Please, is there another way to prevent spam registrations?? That tutorial is too complicated for my limited computer knowledge. I'd like to have something in place before it becomes a problem! Thank you.
