Forum Problem

A few weeks ago i created a fourm on this site then completely forgot all about it. I forgot the name , my password and everything , so i tried to create another forum with my email address but it wouldnt let me as you can only have one fourm to an address .... is there anyway the forum i created first can be deleted or my name and password sent back to my email address ?
