You can make a thread. And add rules. For example:

Banned? Make an Appeal

You can create 1 Account just to make an appeal.
After we verify it and unbanned you.
We will automatically delete the other account.

For appeals;

Username banned: Name who banned you: How long have you been ban: Why did you get banned:
