Donation system

Hello,

I'm trying to code a donation system for my website which will send the information to me along with their donation, for example:

Their name
Their ID
The package they donate for
Additional notes

I've made the form but don't know how to implement it with a donation button so basically what I have so far is the form and it just sends me the data and not the donation.

EDIT: Sorry if I've put this in the wrong section, I'm clueless.

Are you using Paypal for donation??
The form can just mention the amount that they would enter.

If you're using Paypal and want to create a donation button then see this page on the Paypal website :

Just put it in a forum widget or something. The only thing that would be a bit more complicated would be if you want to add it to a more specific place, but that's not very hard either.
