Yes or No?

I am using free forms served by other sites for survey, comments, and raffle games. And I have to resolve to Personal Code to verify that the person who submitted the entry or data is indeed the member itself.
It would be great if we could have an in-built form and tabulated results with the following features:
1. at least 3 fields.
2. has the option for required or optional input.
3. has the option for private and public viewing.
4. has the option for members to allow members to view and/or edit their entry.
5. has the "data added" message.
I guess this would be a great help.
