filling that form is very easy just copy it and paste in your message and where it written YOUR INFOS there write what you want
like this
Nature of the creation : Banner
Creation size (in pixels) : 800 180 what baner size you want
we can't fill that form you have to fill it .
It helps Graphic Designers to know what you exactly want.
