Affiliate Box Help

Hey Everyone,

Here's the deal... I was adding an Affiliation Box on my forum and I kinda deleted some of the code then saved it by accident... can someone please tell me the default code of the Affiliate box without HTML. thank you!

The only code i can remember clear is this:

no thats not it, it is something along those lines tho. its their a way to reset the box? this is what I have in the box but i need the image part...
