How can I create an invite friends form that looks like this:
The sender of the email will be my board? Does anyone know the HTML of this or how I can accomplish it?

This exists already. Go to AP>General>Forum Promotion> Invite friends>e-mail tab.

This ._.
And yeah, just reread your question, try putting them in a div style or table style code and then use the [SEND] as an a href code.

Re: Invite Friends form
Well, if you're planning to use multiple send things in there, you should make them separately and categorize them, for example, if they're for FB purposes, etc.
So here's a bit of an example.
If you're planning on doing multiples, do this:

Re: Invite Friends form
Well, if you're actually thinking of having an outside server for this, it's possible but you have to contact that server yourself to be able to have a preset on where to send this through your page and directly to them.
If you're going to do that in forumotion, the best way to actually do it is via PM.
Where do you want the send thing to redirect anyways? That's the question I need to know before I give you something away that wouldn't help.

Re: Invite Friends form
The send button will simply send the message to whomever the email address that is posted to. For example, if I filled out the form to:
Subject Line: Important, please read
Friend's Email: random@random.ca
Attach a message: yaaddadadaad
[Send]
Once I would click send, the message I wrote would be sent to random@random.ca with the subject like Important, please read. After I click send, it would simply show a html page that would show something like:
You've sent your message! Would you like to go back to _____.ca
This is what I had in mind, is it possible to do this, if so, can you provide me with the necessary coding please?

Re: Invite Friends form
Yeah, that's what I meant. It's kinda like the rankings I did, I had an external software/website to do it.
Anyway, wait for others to answer this because I don't think it's possible unless you contact an external website from forumotion.
