How to Embed a Google Docs Form into an HTML page using HTML Pages Management

Hello, I have searched and cant locate what I need to know.

I am trying to embed a Google Docs Form into a separate HTML page using HTML Page Management but when I paste the embed code provided by Google Docs into the HTML editor nothing shows up but a blank page with header and footer.

Is there something that I'm missing in the code that could make it work or is it not possible to embed Google Docs Form in an HTML page on Forumotion?
