If you are having a problem filling out there application, then you will have to contact google to ask them how to resolve the problem.

When completed and your problem is fixed with them.. they will provide you with a code to add to your forum.. then if you are having a problem getting the code to appear or need to know where to place.. then we can help with something like that.. but as far as what your asking you will have to contact google support.

I am sure it's nothing serious and they can help/fix it for you.
