Create PHP File?

Not sure if this is a duplicate, but I don't think it is.

Is it possible to create a PHP file on your website? Because, I found a contact form I would like to add, but it requires me to also add a PHP file.

This is the form I would like to add.
This is the page I would like to add it on.

I have been browsing for good contact forms, and I don't switching if anyone has a better one.

You can't add PHP pages to forumotion. You'd have to find a (probably paid) host.
