Warning System Help Urgent!

Now I know how to make a warning system and everything but, when I click images list and add the images the do not show up also when I try to modify it, the thing won't let me add anything. This is how it looks turns out the thing is on the wrong side and is causing it to mess up and not allow me to add anything. I can't even view it. A fix on this or a solution would be helpful. Here's the image on how it looks.

1. Figure 1 (Only let's me add images when I first create the list, but then it won't show up and when I try to modify it, it comes up as figure 2

I did this by following the tutorial... If you clicked link two you would see that the add button is on the wrong side causing it to not be clickable

I am using Firefox, I always have... I think its a bug on the Admin CP, in figure 2 the "+" option is located under number... Lol this needs a fix, or if thats not the problem idk what is... I did everything in the tutorial...

Hi, could you PM me a test admin account so that I can see this for myself? It would also be helpful if you provided the images that you were going to use.
