Need HELP with a slider (all the info is in here)

I have a forum and it has a blank html sheet that looks like this: https://i.servimg.com/u/f44/17/34/90/11/htmlpa11.png

I also have a Separate CSS sheet that looks like this:

Now here comes the confusing part... I downloaded a slider for my forum...It came in a general file that looks like: https://i.servimg.com/u/f44/17/34/90/11/fistfi10.png

*The html file html file has this inside of it:https://i.servimg.com/u/f44/17/34/90/11/second10.png

Side Note: I tried to go to the index and simply view the source to copy the code...but the code did not work at all (it tuned into the exact text I put in)

*Inside the PSD file there is this: https://i.servimg.com/u/f44/17/34/90/11/third_10.png

* and I have no clue what the .DS_Store file is.

So the question I have is how do I install this slider into my forum, based on what it has...
