In the users profile scroll down the bottom and click Ban Member.

Has the user been added to the moderators group by the admin?

    Hello,

For the ban issue, try looking on this setting:
Admin Panel > General > Security:
Disallow moderators to ban members : No
    > Save

    For the profiles:
    Admin Panel > Users & Groups > Profiles :
Click the profiles you want to be edited by the mods Edit
Who can modify the profile field value ? : Check the box for moderators
    > Save

AP>Users and Groups>Groups>Group administration. You should have a group called "Moderators", click on this icon Moderation (Baning&Profile field editing) Editer and add the member you want to give banning rights to. "Save"

Please make sure that the moderator group have the permission to moderate on at least 1 forum, you can add this permission on:
Admin Panel > General > Categories & Forums > Perms
Give the moderators group the permission to moderate on the forums

See if this will give them the permission to ban.

Now, is there anything that has been changed on the forum with the template codes, scripts etc. ?
