That has nothing to do with us, but with your browsing.
There can be several reasons, like a change of the IP number between the display of the page and the moment when you filled the form.
You need to check out your connection.
Edit : There's no need to complain also on the Facebook page. The Support Forum is the only place where you can have some help.

After talking through private messages, we determined that it was the email address that was being used causing the issue. After using a different email, the forum was created.
