Thanks so much for your patience!  =)  As for the different form of proof, I am afraid we are unable to accept any other form.  However, if you are uncomfortable with showing your screenshot through a public thread, feel free to PM it to me and I'll be sure to post if your forum is indeed verified.

Additionally, I just want to make sure I understand your request correctly--you're wanting just one image, which would be a thin userbar to go above your signature, and that is all?  The "Creation Type" suggests that you are wanting more than one, but your description detail does not, so I just want to clarify.

You are correct. I just want one. The proof screenshot will come soon, then.

Sorry for the multiple posts, it seems I have no way of editing my posts. However, I wanted to show you guys that I tried this on my own using GIMP2 and the result was pretty good in my opinion.

This is my first completed image on GIMP2, since I am relatively new to the program and still getting used to all of it's features. With that being said, pretty good for a beginner, eh?

Since I already finished creating my request, it's up to you Leah7 if you want to take a stab at my request still. Your results might turn out a little better than mine
