    We are not clear on whether only those who want the update need to apply it. If we don't apply it, do the old settings still apply? We use phpBB2.

    Everyone is getting the updates.  If you have a punBB or phpBB 2 forum with modified templates, you need to do what is instructed in the message about the templates.
