Option / Choices via Pictures

Is there a way to make a variety of pictures and let ppl choose what they want and then Submit it?

Ex:

BlueDragon
    []        

CyanDragon
   []

RedDragon
 []

GreenDragon
[X]


Then when they click send, ittl send the moderator that they chose  Green...
or something like this acualy..
