you must build it or look for some website that provides you with the Html code to use in your forum

We can't really give you the same code as it was made by the owner of the forum but you could ask them if you can have the code's they have.

if there's another chance, how to make the item reward i made is having connected to the form?

There is no automated shop code, you must make your own on an HTML page and then style it with css. You must then manually manage the points for each member
To change members' points go to administration panel ..modules .. points and reputation .. points donation
