I submit other program to check online users, and sometimes it shows different amount....

The 'most users ever online' seems doesn't change even though the site has ever reach more users than the stated one...

Btw most users ever online is based on registered users only? or including guest and hidden users?

Well i do most user online does include guest and hidden users and members.

I have "Guest" viewing my forum contents when I check out the "Who is online?"

However, how can this be possible when I have restricted access to approved members only??

And how or what measures is there to prevent unwanted guests to view my forum content??

Even if you make it so it is under mantenence guests can get on but not see anything and be online otherwise if you made it just for members you would never get any if when people went onto it it kicked them off.

Weather your forum is open to guests to post or not to see a thing they will always show as online as they are still on your site
