Well, jotform.com is pretty good and easy, it's a drag and drop. But, every forum also comes with it's own contact form, you know?

Woops my fault Embarassed . I exactly wanted to show you this torturial. You don't have to fill in any code. At the end you get a long stroke of HTML. Copy and paste it on one of your HTML pages and you have your form.
