How to add a Custom Form Mod?? Empty How to add a Custom Form Mod??

I wasn't sure where to put this, but I think this might be the right place.

I wanted to add a custom form to my forum, but I don't know how. I want it to work like this one here:

Is there any way that I can do this??

In the admin CP under modules you should see HTML Pages, then you could use another company that allows you to make forms and embed them into your site... e.g. Bravenet or MyFreeContactForm Smile

Try out Jotforms. It's a pretty nice site to get your own form.
