You can put it as one of your portal/forum widgets.
If you want it in the Portal:
ACP>Modules>Portal widgets>Structure>Create a custom block>Fill the form>Save
If you want it in the forum:
ACP>Modules>Forum widgets management>Create a custom block>Fill the form>Save
