Creating a Survey

I'm not sure whether I spelled It correctly

I would like to create something similar to a survey - not a topic - something that could be placed somewhere on our Index where members will answer a series of questions, then submit their answers. Then, their answers will appear in a topic only visible for Admins. This will help me to improve my forum's activity.

No, something more appealing from a user perspective. For instance: a widget right below our logo, same width as our logo, and it goes down a bit with a scroll bar on the side, with a bunch of questions and answer fields, once a member has answered all of the questions he then hit submit (or anything else relative) and the answers then appear in a topic only visible to the Admins. Of course, the survey will not appear to anyone who has already answered the questions.

As far as I know something like that would be hard to do. Primarily because you cannot have a forum visible to admins where members can posts and not see; it's not possible AP-wise. You could create a topic with a list of questions, and in the topic tell the members to send it to you via PM. You then share those PMs in a topic under a private forum for staff.
