In my experience, this usually occurs for some reason if there is a ton of BBCode in a post and you use the preview button after editing the content. I have always assumed this was just a glitch, so I just publish my edits and then edit again until it is how I want it. I suspect when hitting 'preview' and there is a lot of bbcode that is repetitive, it somehow gets confused and adds the extra bbcode tags even though it doesn't need to. This is just what I have concluded over the years, although I am not sure if this is actually why it happens.

I have not been able to find another solution to it aside from copying the text before hitting preview so I can paste it back in.
