Online form submission?

I am looking into the possibility of letting my forum users submit a form via the forum.

I run an online football managing game, and would need a form that you could set up with drop down selection menus, so they could alter positions etc etc.

any ideas?

You need to add a poll with two options YES & NO, so other members can vote for this.
