Problem with created user profile

Hello,
I created a new user profile rank thing where I give them a top poster rank if they're in the top 10 & I'm sure I've set the permissions right.. but some of the people that sign up have it automatically =/ What am I doing wrong?

Did you add a default blank image?

I've just deleted it & made a new type called "in an images list". How do I create a default image of blank?
