Warnings bar gone

Hi guys i need a little help with a minor issue . Some of our members have noticed that our members warning bar seems to have gone . This was a item that i installed via here and ive gone through my admin panel today to see if i can locate it to see whats going on but it seems to have completely gone from my forum . Has anyone else had this happen to there sites or has it simply been switched off .
Any advise as always greatly appreciated

Maybe the images got deleted from the hosting site. Please check if the images are available.

Now can anyone remind me how to install some new ones as i honestly can not remember how i did it 1st time around
