I need some help making a fillable page

well im trying to learn or find help on how to make a working fillable page that people could of course fill out and it would be sent to my email address.

on my site, i use it as a contact form, and a mod application form, and also a car of the month entry form.

but ofcourse i have edited the html code to get rid of the advertisements and the other non sense.

ofcourse if i leave it up ill get a bunch of spam crap from the little kid members on here
