What is guest avatar??

It's an avatar for guests by default settings by the founder.

but when the guests are not allowed to post, then where will it appear??

It won't appear at all if a guest never posts.

Yes it depends on the permissions you give for the guest, whether to allow them to post or not.

That only appears when somebody was once registered on a forum and posted and suddenly the account gets deleted and the posts of that user remains. I've had that on one forum I am on.
