Problem with capcha directing to registration form

Im having problem directing my captcha to the registration form. I've already follow the tutorial that was given from this site

it suppose to direct me and i did all it said. Can someone help me. This is how i did my new html as it says

did i do something wrong? just need to make sure...i've been on it for days...
