How do you make a customized Menu?

Ive been trying to create a link in my navagation bar that says Who's online but (i know where to make a customized menu) but whenever i try to put a link in it sayys 4004 Not Found and Return to index. PLEASE HELP!!!

URL GOES HERE is the URL of the option. Like if you click "Profile", the URL will direct you to the "Profile" page

The URL of the thing comes first, which connects to "Topic Being Watched".

So URL first then "Topic Being Watched". To know which one first without referring here, URL then you'll see target=_blank> XXXX. Then it's the correct format.

You create a custom module and paste the code in the field.
