How to warn a user without using the bar system

I want to know how to give a user an infraction without using the bar system.
I know it can be done, because I've seen it on a few forums before.
Here's how it looks like next to a post-
Name: Eg. Blargg
Rank: Eg. Mod
Avatar:
Title: Eg. Me
Infractions: Eg. 1
Warning bar:
Posts:
.................etc

On forumotion its not possible.

As sanket said, there's a TRUE PHPBB Mod for it, but because were not using true phpbb, you can't do it that exact same way.

You could do it in a form of text, as you have posted above?
Admin panel >> Users & Groups > Profiles > Create new.
Name = Infraction.
Type = Text
Obligatory = No
Who can edit = Your call, mods + Admin or just Admin? (Don't allow members to edit this)
=> Save.
