Hello, I am trying to add this code to my forums, this is what I'm trying to add and off this website: http://thecodeplayer.com/walkthrough/jquery-multi-step-form-with-progress-bar

I've done everything it's said to do, I've added the HTML to the HTML module, CSS in Display and Javascript in Javascript management however whenever I preview the HTML it comes up with this:

EDIT: After selecting Yes to "Do you wish to use your forum header and footer ?" It seems to be working however it doesn't look quite right?
