Userpage with Warning Meter/other

I was wondering how can i put in a meter like the Warning Level we have,and how can I put in that and other meter with different name,and how can I raise it and lower it.Also can I make an option where people can report post to Admin/moderator ?? Lastly how can make somethin like this.
