Question about Personal Domain Name.

I am about to purchase a Personal Domain Name for my forum but before I finish of the process I am not completely sure how to fill out the form.

The first 2 lines are Name, and First Name. For this do I put the name of my site in Name and my first name (Patrick) in the first name or is there another way to do it.

My next question is do all the fields need to be filled? I don't have a fax number and my site is not an organization so I don't have to fill in these fields right?

My final question is why do I need to submit so much personal info. It asks for my complete mailing address and phone number. What is this personal info used for. I won't be getting any phone calls or junk mail will I?

If anyone can answer some of these questions I would very much appreciate it.

Yes, fax # & organization are not required, if you don't have any.

Thank you very much. This is very helpful. It said i needed to fill in the FAX and organization so I just wrote N/A.
