Staff Widget Coding Help? -Take Two

So basically like the title says, here is the exact problem
We got a new moderator and we were also demoting someone so I had to make a new group however that happened no matter what. Like if you hover over the image it still gives you the persons name, like Deadbolt or Lexa but the picture isn't what it's suppose to be. If you click on the site link you can see what the widget is suppose to look like.
Here is the code I'm using. So if someone could tell me what I'm doing wrong/or fix it that would be awesome.

Bumping cause it's been like two days and I've tried a bunch of different fixes and it doesn't seem to fix the image error.

When I hover over the images, I see the link to the profiles, not usernames.
