For the lay-out please visit your 'ACP/Display/Structure &Hierachy' and choose Split categories on index : Medium or None.

The look of your forum can be changed either through your ACP/Display/Pictures & Colors/Pictures.
or with CSS code through ACP/Display/Pictures & Colors/Colors/Cascading Style Sheet.

Also a category will only appear if you have a forum attach to it.
