Adding a Questionnaire to the Registration Process

Is it possible to add a questionnaire that new member would fill out while registering?

Yes its possible,

ACP --> Users and Groups --> Profile Fields --> Click this button --> Fill out the boxes you want and all you need to do is tick the box "Necessarily filled ?" to yes and it will show up on the registering page.

Yes its the only way.

So just make sure you tick the "Necessarily filled ?" yes box and it will show up on the registering form.
