Reputation Improvement problem

NOTE: I was not sure if this would be a design or a script problem, so I went with design for now.

We have been using the Reputation Improvement javascript for phpbb3, found here: Reputation Improvement thread

It worked great until about a month or two ago. Now it looks like this:

Instead of this:



(Ours has tiny + and - on the far right, instead of the thumb icons.)
We have made no modifications to the code, we simply copy/pasted what was asked of us in the thread. Any help would be greatly appreciated!

Thank you for contacting the support forum today.

about 2 weeks a go we had a small problem with a code and had to do some change to our systems.

Some forums had to have the Javascripts turned off so can you please look at the following settings.

ACP >> Modules tab >> HTML & JAVASCRIPT >> Javascript codes management >>
Enable Javascript code management : YES then Save

this should fix your problem.
